AD SALES SHEET

This form is used if you have someone, (or yourself), wishes to place an ad in any of our programs.  All your questions about ADS can be found on this form.

PHOTOGRAPHY RELEASE FORM

This form is necessary because it gives us permission to use a publicity photo WE take of our cast members.  Photos may be used in Newspaper, television, or future advertising for PREMIERE or THE BROADWAY THEATER’S

TICKET SALES SHEET

All children cast in our shows are required to sell 10 tickets to the production.  This form is given out the first day of rehearsals, but, many people fill out and need another - Feel free to download this sheet and keep selling tickets

Every time you participate in one of our productions there are certain forms we require.  There are also forms that you as the actor, (or parent of an actor), will need.  So we have provided an entire page of the ‘forms’ we use all the time.  These forms will ALWAYS be given to you on the first day, but, if you lose one, you know where to print it out!
